I'm just doing this for fun and ideas, I'll clarify things if something seems out of place, but I don't pretend that I would even allow this in one of my own games. I can also describe them in way more words than any book would do since I don't care about page counts.
Tried to only choose spells from the Player's Handbook and didn't really worry about how useful they would be to a PC or not. Found the Fate domain in the Spell Compendium, tried to make the Supreme domain different from the Inquisition or Retribution domains in there.

Supreme Domain
Granted Power: 1/Day grant yourself or another target a +4 enhancement bonus to one ability for a number of rounds equal to your cleric level. Alternately, you can grant a +2 enhancement bonus to each of the target's three physical abilities, or each of the target's three mental abilities instead.

Granted Power: You cast Domain spells at +1 caster level. This stacks with domains that grant caster levels, like Healing, Good, or Evil.

Granted Power: You cast all non-divine spells at +1 caster level. (Too powerful? Add 'if they are of spell level equal to or less than your cleric level.' Me, I don't care.)

Rebirth Domain
Granted Power (simple description): Once per year when you die you are reincarnated (as the spell but with no loss of level or XP and you may choose the result immediately above or below the rolled result if you wish) at a temple or holy site of your religion or deity. This functions across planes assuming your soul isn't trapped or bound.

(Complicated description:)
Immediately at your death you have the option of imparting one or two sentences in any language you could speak at a normal volume which anyone in the area can hear. This speech might trigger contingency spells or command servants or automatons but it can't trigger magic items. Instead, your spirit can opt to deliver a one or two sentence message to the mind of any one person whom you knew reasonably well. This fails if the person isn't on the same plane where you died. Typically these messages are used to reassure allies or tell them where to find the reborn character, although they could also taunt your killer.

Rebirth does not occur for those who die of natural causes such as old age. Although that should almost never happen, since most clerics should be able to reincarnate into a healthy young adult body well before they reach that stage. As such, they also tend to avoid old age's physical penalties while attaining the wisdom benefits. You should use their original race's aging table to keep track of when their mental stats increase, though they should only get the benefits once for each age category, regardless of how many times they lose any physical penalties which should occur at their current body's aging rate. Although old age is practically the only natural death for most creatures, a phoenix's immolation of itself in extreme life-threatening situations or at the end of it natural life would be considered an example of a natural death. Fortunately for them they have their own method of rebirth.

The once a year restriction is linked to the character's soul, which means if he or she is reborn and then time travels one or two years into the future, Rebirth would not work. Similarly it would fail if they time traveled back to a point before they died. If they are in Temporal Stasis or a similar effect that causes them to be out of time, then no time has technically past for them. If they move to another plane or dimension where time travels more quickly, then they benefit from that. For example, being on a plane where one year passes for every day on the Prime Material. As long as they spent the year there before returning, it might seem that they were able to be Reborn after only 48 hours (since you would have to spend at least a day and 1,000 gp. rededicating the site for your rebirth after your return. See below.) Of course, doing this will also mean you are entitled to almost no significant monetary or equipment assistance from any temple you appear in, since it will have only been about one day since they last gave you aid.

At the time of your death, your body is removed from existence in a divine glow, including severed limbs, though not any superficial leavings that came off prior to death like pulled-out hair, spilled blood, or knocked-out teeth. This occurrence also prevents the character from becoming undead in almost any circumstances that don't immediately bind his soul to his body and very few characters with rebirth would find themselves bound to the site of their deaths as ghosts or similar undead since they should know they will be reborn. If somehow the soul is trapped, the Rebirth effect waits for the destruction of their undead form or the freeing of their soul, assuming their rebirth site is still valid when it occurs. This only happens if the character would be reborn at the time of death, not if they were reborn 8 months prior and then became an undead creature that rampaged about for a year or two before being destroyed.

Any equipment you possessed is left behind at the site of death with the exception of one holy symbol which appears with your new body. Your new body reforms 24 hours later garbed modestly in a robe, shroud, or burial clothing with your holy symbol or a completely new, though inexpensive, holy symbol if you didn't have one when you died. This site is prepared by performing a day long ritual and expending 1,000 gp. worth of precious oils, herbs, and materials

The site remains prepared permanently until a rebirth occurs, at which time a new ritual must be performed expending the proper materials, or until the temple or site is destroyed or defiled in some way. In the latter case, a new rebirth site must be found or else no rebirth will occur for anyone attuned there, they will just die with no effects of this power occuring. Unfortunately, there is no indication when a rebirth site is compromised, so it's good to check on it regularly. A rebirth site must be a holy site of significance to the deity or religion or at the very least a modest temple, a shrine is not sufficient nor is even an altar that has been hallow/unhallowed someplace. Suitable holy sites often do have shrines placed at them however. For instance, the god Fharlanghn has very few actual temples and a lot of roadside shrines, if Fharlanghn had the Rebirth domain, his followers would likely have to find a site holy to him to be reborn at.

Typically rebirth sites are at major temples instead of holy sites for several good reasons. First, the highest ranking priest is alerted when a rebirth is about to happen and not every holy site has priests on location consistently. Second, a reborn character can prepare ahead of time by storing extra gear in temple vaults set aside for the reborn for when they return. Things can be stashed at a holy site, but might not be quite as secure or protected from the elements. Third, temples help reborn souls by furnishing them with suitable gear and clothing, even magical gear if the character is of suitable level and the temple is of suitable stature. This all depends on how well the character has been helping the temple, so it's always good to send back tithes and make donations beforehand. These may not be the same items that the character left behind at the scene of his demise, but they should be useful in starting this 'new chapter' and rebirth.

Due to this, it should be considered a spiritual failing to strive to reclaim old gear and possessions without a very good reason. You have been reborn and should not anchor your new life with rubbish from the old one. You can seek to complete an unfinished quest, or contact old friends, since these can be approached in new ways with your new body. You can try to recover letters you had at the time of your death if the intent is to deliver them, or you can recover a needed key or item to complete a quest, but going back for your magical sword and gear is not acceptable. You will likely need an Atonement if you do retrieve them, no matter how awesome and powerful they were, even if one of your quests involves fighting a tough bad guy or dangerous magical beast and it would really make it easier. Even taking back a family heirloom should be discouraged since such things are meant to be passed down at times of death to the next of kin, and you had your allotted time. They should be promptly sent off or delivered to avoid contaminating your new life with the old. Seeking revenge for your death on your killer may be fine depending on your deity but any of your old retrieved equipment and gold should be donated or tithed away, but not given to other PCs if they didn't already loot you.

It's really hard to come up with a list of spells that embody "Rebirth" since I have so little to go on for the aspect but a name.

3rd Magic Weapon, greater (in case they do come back someplace and need a magical weapon quickly.)
4th Reincarnate (It had to be in there.)
