Make sure you are using the latests version of this adventure. It had clearly been written before the rules were set in stone, and some things (like elites)
and later printings have different stats for iron tooth.
Remember that as DM, you can fudge things a bit. Don't be afraid to pull punches if you think you're heading towards a TPK (and you don't want one.)
Lastly, if the party does get overwhelmed, remember that the rules for knocking targets out in 4ed simply that you say "I don't kill him" when a target goes down. It wouldn't be a bad idea to have the Kobolds (Iron tooth) try to take the players hostage here. That way if they all go down, you can do some sort of "escape" thing. At as such low levels they won't have a lot of gear to replace, so this might work out fine.
That said, some adventuring parties have been known to walk all over these encounters.
